In a message window there isn't an accessible way of saving attachments. If a shortcut key existed for the "Save All" button (or it's fuction), the accessibility of saving attachments would be total.
As it is now, only using Flat Review (in Orca) is it possible to reach the button.

I see 'a' as mnemonic for save all option in 2.29.90
