This may be related to #743724 but is not quite the same.

Having opened the first terminal window in a new GNOME session by picking the icon from the panel on the left of the activities view if I then right click on the background of that terminal and choose "New Terminal" it opens the new terminal in a tab even though I have the preference under "General" set to open new terminals in a new window.  If I then close the new tab and choose "New Terminal" again, this time it opens in a new window and all subsequent new terminals open in new windows.

Could you please check if you're experiencing the same as bug 794402?

Yes, it is.  Glad to see this has been changed to Ctrl-Click only.  I take it the fix will be in a point release, like 3.28.1?
