I reported missong "Copy medium" icon in k3b burning software to kde team, a k3b maintainer said this is a Gnome icon theme issue. See the bug report from kde bugzilla please.

Thanks for reporting the issue. I'm afraid this is a custom app icon, however. The purpose of Adwaita is not to provide a library of icons for 3rd party apps.

I am surprised k3b falls back to not showing any icon, because the generic icon naming lookup just falls back to either the parent theme or a generic icon name.
