I am using gnome-shell from git master and pulseaudio 9.0 without any patches.
To reproduce:
1) Change volume to 100%.
2) Log off gnome.
3) Log back in.
4) Press volume+ on your keyboard.
5) Notice the slider is empty even though volume is actually 100%.
6) Press volume- on your keyboard and the slider will sync correctly.
7) alt+f2 -> r and notice that the on-screen volume changer is out of sync again.

In the meantime, the volume control top right of the the window is always correct. Only the onscreen volume control that pops up in the middle of the screen is incorrect.
