Here are two yes/no dialogs that I found. There might be more.

The "are you sure you want to delete this file" 
and the "are you sure you want to empty the trash".

This is a HIG compliency issue so I attached that keyword.

I'm pretty sure the hig specifies to avoid using yes/no no/yes dialogs
at all cost.
