3) What is expected to happen is when one opens the following attachment, scrolls down to the bottom right, to the right of "Nom et signature" click the field and an pop up opens to insert a picture file, as it does in Adobe Reader https://bugs.launchpad.net/ubuntu/+source/evince/+bug/426145/+attachment/705192/+files/form_fiche_evalrisq.pdf .

4) What happens instead is this is not a clickable option. First reproduced in Ubuntu 9.04 evince 2.26.1-0ubuntu1.

It is not trivial.  The document has JavaScript.  Poppler, the library used by Evince to render PDF documents does not have support for JavaScript.  Therefore, it has to be implemented there first before anything could be done in Evince.
