but the form didn't actually submitted, without any warning message.

make the form a partial

Add the form into a partial so we can add some error correction without having a messy page.

This patch isn't solving the problem, but if it is okay to add a new page, I can work further on it. Otherwise, I should fix it on a single page, which isn't so nice.
