The close and minimize buttons disappear whenever the windows is maximized. This bug appeared after the update to Gnome 3.12. 

My Evince version 3.12.1-1

Gnome-Shell version 3.12.1

There are no options apart from help in the application menu.

Thanks for reporting an issue.

Evince does not handle the window decorations (close/maximize/minimize buttons). It is the window manager.

Regardless, I am closing this bug a duplicated of the one that involves porting to GtkHeaderBar.
