When running contacts for the first time, I am presented with an option to use an online account or a local address book. There is no help menuitem in the app menu at this point (nor anything else except "quit") and no help button in the dialog.

I am accessing this dialog by right-clicking a contact in empathy and selecting "Information".

1. What is help menu item? 
2. What is app menu? Is it the menu that appears when one right clicks on the program name in GNOME Shell bar?
3. What do you mean by "no help button in the dialog"?

Yes.
