PDF form not working

The 'Student's Name' field doesn't work correctly. For example if I enter 'Test' in the field it is displayed as '5FTU'. Using evince 3.4.0 and poppler/cairo (0.18.4) in Fedora 17.

I can confirm this with poppler 0.20.3-2ubuntu1 evince 3.5.90-0ubuntu2
I get the following output in a console after I press 'Enter' in the field: Syntax Error: Unknown font tag 'KozMinPr6N-Regular' Syntax Error: Unknown font in field's DA string Syntax Error: Missing language pack for 'Adobe-Japan1' mapping Syntax Error: Missing language pack for 'Adobe-Japan1' mapping The text is not displayed, but it is there because it is shown if I go to the field again. FWIW, Acrobat Reader 9 says: "The Japanese Language Support Package is required to display this page properly. Under the current configuration, this resources is not available. You can download it from http://www.adobe.com/go/acroasianfontpack". However, after the message I can enter ascii characters and they are shown. So, Evince should at least be able to do that. Anyhow, this seems to be a problem in poppler.

The text is not displayed.
I noticed this is the same as Bug 655587. Entering "Welinder" in any text field produces "8FMJ0EFS". *** This bug has been marked as a duplicate of bug 655587 ***
