Two Index Issues on PDF with Doxygen 1.8.1.2

On the PDF index,

1) The Class Index entry is duplicated, one stands for Class Index while the other corresponds to Class Hierarchy

-> Rename the entry for class hierarchy as Class Hierarchy

2) The Index at the end of the PDF isn't referenced in the index

-> Include an entry on the PDF index for the Index

Seems like you submitted this bug before: bug 679963. 
Please update that bug with new info instead of submitting a new one!
