I'm in xterm, code code code, commit. Emacs pops up for me to write commit message. C-x # to save the commit message, automatically close Emacs and return focus to xterm. Alt-tab then will choose xterm while I expect it to choose the next app/window.

I don't know if there's some issue with sorting. An easy approach is to avoid focused window/app as the first chosen app. Unfortunately I don't know how to know which window has focus.

Not completely sure about the duplicate marking (try the patch there), but I think it's the same since it involves alt-Tab going to the same application and Emacs.
