If your program does not have an icon (.desktop file?) you can use it to open files within nautilus. For example, how would you open the video file in the screenshot with mplayer?

Yes, a .desktop is required in Nautilus 3, but no you don't actually need an icon with your .desktop.

Well, so this either should be closed as notabug or changed into something like:

"Open with does not allow custom commands to be used"
