I had a mismatch between g-s-d and shell, such that on shell startup:

GLib-GIO-ERROR **: Settings schema 'org.gnome.desktop.interface' does not contain a key named 'accessibility'
aborting...
Shell killed with signal 6

Fallback mode never kicked in... it just kept retrying to start the shell.
