pdf form with fields filled by evince but not readable by acrobat reader 9.2

I must fill in data fields in a form generated by Adobe Acrobat and return them to a colleague. Whereas the filled forms are readable by evince and by gimp, when I open the filled form in acrobat 9.2 the fields are blank.

I am attaching on of the filled forms.

This a bug in poppler. I've just forwarded bug #614740 to freedesktop bugzilla because it contains more details about the problem.
