Merge page setup and print dialogs.

Currently if you want to print on custom paper you have to open page setup dialog each time (most application don't save settings in this dialog)

And only then select 'print...'

It would be much easier to do both in single dialog.

And on top of that fspot has no page setup dialog, so how I select page type there?

That applications don't save print settings is not a gtk bug...
