I have found  duplicate markup in the docs concerning the entity &app;. When entities are expaned appears a double <application><application>gcalctool</application></application>. 

Markup should be on the entity or on the text, but not in both at the same time.

Thanks for reporting this.
However, the whole of this user manual is out of date, so there's not really any point to fixing this now. I'm marking as a dependency of bug 309210, the Big List of stuff to fix.

Looking at the last gcalctool help documentation, this problem problem
no longer seems to exist. Presumably it was a more global problem then
just with gcalctool. Closing as FIXED.
