Steps to reproduce:
0) Find a sidebar link
1) Click it
2) Tools->Extensions
3) Uncheck "Sidebar" to unload it
4) Click "Add Sidebar" in the still open dialogue

Actually my previous comment was because of a mis-reading, we should just
destroy all the "Add Sidebar" dialogues when the extension is unloaded.
