References to named regions are lost (changed to #REF!) when rows/columns
are inserted, even when they are outside of those regions.  Additionally,
the named region reference is not always updated.

Example:

- Type 1 into B2.
- Select B2 and go "Insert->Name->Define".  Type "my_cell" for the name,
and "B2" for the expression.
- Type "=my_cell" into A1.
- Insert a column before column B.

Result: 

- The contents of A1 change to #REF!
- my_cell is still set to B2, not to C2.

Expected result:

- A1 still contains "=my_cell", and still displays 1.
- my_cell is defined for expression "C2".

Reproducibility:  Always, although I think that sometimes the expression
for the named range is actually updated, although it is normally not.
